Complete Medication Review change

Set a time to take a deep dive into your medications, over-the-counter products, vitamins, and herbal products.  We will review your regimen prior to your appointment for any drug-drug, drug-disease, or drug-supplement interactions.  Reviews include education on medications, guidance on medication timing, addressing of any concerns, and disease state education.  We look for ways to lessen pill burden and educate on any warnings or concerns with long term medication use.   

This service is covered for some patients with Medicaid, SeniorCare and Medicare Part D.  We will let you know if you meet the requirements under your plan prior to your visit.

  • Pharmacist Consultation

    • Telephone Consult $30
      Pharmacist Consult on over-the-counter meds, supplements, and medications.  
    • Herbal Supplement Interactions $60
      Pharmacist review of interactions between prescription medications and supplements, herbal products and CBD.
    • Medication Check Up
      Medication check ups are scheduled around regular med pick-ups (up to 1 time per 3 months) to allow our clinical staff to sit down with you to review your medications, check blood pressure, review any outside labs as needed and talk more in depth to you about your medication goals, concerns and discuss any side effects.  This one on one time helps us find any issues before they become serious and help patients work towards their personal health goals.  This service is covered fully by Medicaid for patients with certain medical conditions.
