Must be at least 2 months out from last COVID vaccine and age 60+. If you do not need a RSV vaccine, please go back and schedule an appointment for just a Moderna vaccine. If you are uninsured or underinsured, Ballweg Family Pharmacy is participating in the Bridge Access Program that provides the Covid vaccine at no cost. If you think you qualify, go to https://www.covidaccess.com/ to register for the program. You must complete this online registration, print out the information provided and bring it with you to your appointment. We will not be able to give you a vaccine until these steps are completed. |
